Thank you for registering your athlete for sports at Milan Middle School and/or Milan High School. Please be sure to complete and sign all forms and set up your athlete's sports physical as soon as possible. Athletes are not allowed to participate without a current year sports physical on file. A current year sports physical is one given on or after April 15th of the previous school year. Also, it is very important that you have your athlete read and sign all athlete forms. Athletes must have all forms signed and a current year physical on file in order to participate.
